Study entry process

If you are interested in participating in the study, please email us at the study email:

1. We will send you a consent form by email.  You need to sign it and return by fax.
2. You will need to obtain a copy of your hospital discharge summary and mail it to our address (we will send you an envelope once we receive your consent form)
3. Once we have those we will schedule a time to do a telephone interview.  I (Dr. Robert Centor) will do all the interviews.
