I really enjoyed this lecture, thank you very much. As a General Practitioner, Undergraduate and Postgraduate teacher in the UK, I am the closest equivalent to the General Internist in the UK NHS.
You have put a lot of what I feel about guidelines into words of some erudition (although I do quote Einstein and Nietzche as well).
I prefer to consider guidelines as Guidance: that helps me a lot.
