Long morning, we were post call and had 8 new patients. The team did a great job!
I saw one patient whom I had seen in 2000. The VA EMR is a wonder, I could read my old notes.
New patients on Friday have longer length of stay. Our VA is excellent, but we still cannot get much done on the weekend.
Infections dominated our admissions. Antibiotic decision making is challenging, and a skill that hospitalists must constantly develop.
Several days ago we admitted a patient who I transferred to the ICU within a minute of seeing him. He did require intubation and pressure support. We still do not know why he has a septic presentation – all cultures are negative thus far. The good news is that our unit team has done a wonderful job, and the patient is recovering. We just do not know what he is recovering from!
We internists love making precise diagnoses. We praise internists when we call them "great diagnosticians". This patient may represent an undiagnosable success. While we would love to have a precise diagnosis, our highest priority is helping the patient recover. Some patients become very sick, yet we do not know why. If they recover, we are very happy.
