Being a must read blog

Well, I am clearly excited. Forbes magazine has me on their short list of must read health blogs. Must-Read Health Blogs
I assume that some new readers are dropping by this week. In this post I will mention the issues that have me ranting in fall ‘09.
1. Inadequate access to good primary care. This is a multi-faceted problem. We have a system that has driven excellent students away from primary care. Our system has made this option much less desirable. As I have written recently, I do not believe that we can fix this problem unless we have a different payment model. Fee for visits has so many problems that I fear it cannot be properly fixed.
2. Funding coverage for uncovered patients. We already pay for their care in the ED and the hospital. We should pay for their primary care and decrease their ED visits and hospitalizations.
3. Time – physicians need more time spent with each patient. Our payment system discourages spending time.
4. Making training adequate and complete
5. Avoiding the performance measure gurus – performance measure incentives have many unintended consequences. They do not define quality, but rather represent a small component of quality. Using performance measurement to assess quality is equivalent to assessing a car only from a picture of the tires and the exhaust pipe.
I hope that I am irreverent. I hope I make you think. Sometimes I should make you mad – if you are mad and that causes you to think, then I have succeeded.
