I wonder if readers have seen patients or physicians repeat mistakes and not consider the possibility that they have missed the underlying diagnosis.

Replace insanity with idiots and you have just described the current rape and pillage of the American tax payer by the federal government.
I think that it perhaps applies to the management of patients who are a diagnostic dilemma- particularly those who are an extended "pyrexia of unknown origin" and get a urine culture, blood culture and CXR on a daily basis for 3 months.
