I am not advocating indiscriminate use of antibiotics for viral URIs, but it has always seemed to make sense to empirically treat cases of pharyngitis that have the potential for producing an abscess and are associated with a toxic appearing patient, even if the rapid strep test is negative. Call me old fashioned, but if I can prevent one case of Lemierre’s I would be happy.
