Thoughts on EMRs

I am currently attending a meeting on EMRs. This has retard my blogging, but I have a short time today to share one major concept. One speaker in discussing EMRs suggested the following stages of adoption:

  • Substitutive
  • Innovative
  • Transformative

Substitutive refers to just turning our paper record into an electronic equivalent. This certainly works, but may not really add much – and detract in terms of physician data entry.

Innovative means being able to do things not easily doable with a paper record. This might include drug interaction alerts, or dosing alerts (when patient has an elevated creatinine). It might even be as simple as beinig able to view lab data on a graph.

Transformative is the state which we would like to achieve. This state implies thinking outside the box and developing new ideas about health care delivery and quality assurance. Only when we have data in computerized form, can we start to transform health care. And we must for the good of patient care.

We’ve had an EMR for 3 1/2 years. We’ve made the substantive & informative changes.

Substantive – All records are accessible by everybody at the clinic, hospital, & home. This alone helps just for accessing patient info off site from home & hospital.

Informative – we have review screens, e.g., all relevant diabetes care items appear on a flowsheet showing recent A1C’s, BP’s, LDL’s, flu shots, etc. We can track health maintenance & set up reminders.

Starting with the transformative with quality of care tracking – I can tell you our clinic wide A1C rates, flu shot rates, etc. Some transformative issues will be held up by our reimbursement scheme, e.g., most plans, Medicare, & Medicaid won’t pay us for providing care & answering complaints online/email.

As for transformative changes – a few things need to happen first. We need true standards for interoperability and portability of EMRs. The real transformative benefits will accrue to the whole system rather than directly at the practice/patient level.
I have a plan to get the players with money (pharma/lab/medical device corp.) to pay for EMR adoption by small and/or rural practices.
I’d really welcome a discussion on barriers to EMR adoption and what we can do about it.
